Please fill out this form to notify JRA of the launch of your survey. We will then email through report viewing details so you can log in to our website and view your responses as they come in.

Please note that we will only send report viewing details to people authorised to view your organisation's survey reports - if you are not our main contact person, we will need written authorisation from that person (e.g. an email) to give you access.
